The player/goalie parent or guardian must read, understand, and agree to fully abide by the terms of this Wild Wolf Hockey Waiver Agreement.

 

I acknowledge the inherent risk involved in ice hockey, including those that take place both on and off the ice. I realize that those risks include falls, equipment failure, bad decision-making, strains and aches. I understand that there are also unforeseeable accidents that can occur and, on my behalf or of the participant I assume all risks associated with such accidents.

 

I agree to assume all risk of personal injury including Paralysis and death that may occur while I or the participant is on or off the ice at any time. I hereby release Wild Wolf Hockey, its owners, directors, trainers, players or goaltenders from all liability for any such personal injury that may occur. This release even extends to injuries that may occur through my negligence of Wild Wolf Hockey, its employees or other parties released.

 

This release applies to and binds my personal representative, heirs and my family. If I or the participant I am waivering participants in the hockey drills, I make this release and these representations on his or her behalf as well as my own, and I agree to assume responsibility for his or her safety. Further, I agree to indemnify and hold harmless Wild Wolf Hockey and any other parties released, in the event a minor member of my family sues them or any one of them. I understand that this means I will pay my fees, costs, and charges incurred by Wild Wolf Hockey or any other party released, including attorney fees.

I or the participant I am waivering is fit. I know no medical or health reason why he or she should not participate in the activities, or drills that take place at Wild Wolf Hockey

 

Any disruptive players will be sent directly off the ice and asked not to return, players will be held accountable for their actions. Every player is expected to be respectful and courteous towards all instructors, players, parents, and arena staff.
 

I understand that this document is a contract. I have read it thoroughly and understand the terms. No oral representations or statements or inducements have been made to me that change, alter or modify anything within the written document. I agree to the said terms outlined, I sign it of my own free will. I also understand that this contract is severable, in other words, that if any part of it is held by a court of law to be unenforceable, the rest of it shall survive.
